Advance Care Planning Services

Have you ever thought about what might happen if you needed to be admitted to a hospital and didn’t have the capacity to make decisions regarding your medical care for some reason? Do you have views or preferences about what medical treatments you would choose to accept or decline that you would want known?

Advance Care Planning (ACP) is a process that supports you at any age or stage of health in understanding and sharing your personal values, life goals, and preferences regarding current and future medical care. The goal of ACP is to help ensure that you receive medical care that is consistent with your values, goals, and preferences no matter what your capacity.

CoVision offers the support from one of our experienced Nightingales to help you prepare your own ACP.  This includes a telephone assessment of about an hour where your values and preferences will be discussed and you can ask questions.

Following your ACP Interview; your Nightingale will then prepare your Advance Care Plan specific to your State/Territory’s requirements. A completed copy will be forwarded to you by email and once finalised a copy will be sent to you by post along with a laminated Carry Card to keep with you in your wallet. You will also be provided with detailed instructions on what you need to do with your ACP, who should receive a copy, and what to do if your decisions or circumstances change, and your ACP requires updating.
